I authorize Vickery Insurance Agency, Inc. to initiate either an electronic debit or to create and process a demand draft against my bank account on or after the date I submitted my payment online for the amount that I entered in the "premium payment" box in addition to a 2.9% (VISA, Discover, Mastercard) or a 2.87% (AMEX) carrier charge incurred by the agency in an effort to process your payment. I also authorize Vickery Insurance Agency, Inc. to include a $3.00 processing fee and any other fees that may apply to my account. I acknowledge that the origination of ACH transactions to my account must comply with the provisioning of United States law.
